It’s very common for Kentucky providers to reach out for insurance credentialing and realize they don’t know their Kentucky Medicaid ID number — or aren’t even sure one exists.

Your Kentucky Medicaid Provider ID is not publicly listed and isn’t something you can easily look up online. If you were enrolled in Kentucky Medicaid at any point, the ID must be requested directly from Kentucky Department for Medicaid Services (DMS) by the provider. For group practices or organizations, this request must be made by the owner, board member, or authorized representative.

What Is a Kentucky Medicaid ID Number?

Your Kentucky Medicaid ID number is assigned after your Medicaid enrollment application is approved. Once issued:

  • You only need to enroll with Kentucky Medicaid once

  • The ID remains associated with you unless the enrollment is terminated

  • You must have an active Medicaid enrollment before credentialing with any Kentucky Medicaid Managed Care Organization (MCO)


This process applies to existing Kentucky Medicaid providers who are unsure of or cannot locate their Medicaid Provider ID number.

How to Request Your Kentucky Medicaid ID Number

This is a new process

To request your Kentucky Medicaid Provider ID, follow these steps:

  1. Complete the Medicaid ID Request Form(This must be completed by the provider or an authorized group representative.)

  2. Email the completed form to: medicaidpartnerportal.info@ky.gov

  3. Wait for follow-up: A Kentucky Medicaid representative will contact you if additional information is needed or once your request has been processed.


Important Things to Know About Kentucky Medicaid Enrollment

Understanding how Kentucky Medicaid works can prevent delays and credentialing issues later on:

  • Revalidation is required every 5 years-Failure to revalidate can result in termination of your Medicaid enrollment.

  • Maintenance requests are required for updates-Any changes to your enrollment — such as demographic updates, license renewals, or ownership changes — must be submitted as a maintenance request.

  • Kentucky Medicaid enrollment comes first-You must be actively enrolled with Kentucky Medicaid before you can credential with any Medicaid MCOs.

  • Group practices require an enrollment-Group practices must complete:

    • An individual provider enrollment (approved first), and

    • A separate group enrollment application

  • Keep contact information current-If an employer or organization completes your enrollment on your behalf, request that your personal email address is included. This ensures you receive important correspondence directly from Kentucky Medicaid.
